When agents propose multiple tool calls in a single model response, all tools get added to the ledger as 'pending' before any execute. The previous implementation assumed sequential execution and updated ledger['tool_calls'][-1], which only worked for the last tool in the batch.

The previous implementation assumed sequential execution and updated ledger['tool_calls'][-1], which only worked for the last tool in the batch and never updated the pending tools before it.
